Having trouble with user profile tutorial 2

Hello,
I followed all the instructions for user profile tutorial 2 and it almost went well if only I could see the fields to edit my profile. I see only the questions with no where to fill out and when I submit, I have a profile with no information but my names.

Any one out there to help me out?
